Some International students, in particular Canadian students, may need to submit an Enrollment Verification form for government funding applications, such as:

• Ontario student Assistance Program (OSAP)
• Alberta Student Aid
• Student Aid BC
• Other provincial or government sponsored aid programs

If you need to work on this, please follow the steps below

Step 1: Download Your Financial Aid Verification Form 

1. Log into your provincial or government financial aid portal (e.g., OSAP Account Portal, Alberta Student Aid, or StudentAid BC).
2. Go to your active full-time application.Complete all student-required sections on the form before submitting it.
3. Download and save the document as a clear PDF file.

Note: If you receive a physical paper copy of your funding certificate or verification form by mail, scan the entire form into a high-quality PDF.

Step 2: Email the Form to BYU-Idaho Student Records and Registration Office

Email Canadian Permanent Fund Dividend forms to [email protected]. Forms that need to be signed must be sent in PDF format from your BYU-Idaho student email. 

Include the following in your email:

• I-Number
• Name
• Semesters for which the form needs to be completed. Include the number of planned credits for future semesters.
• Anticipated graduation date
• Other helpful information

Step 3: Check your BYUI Email

Please allow 1-3 business days for the Student Records and Registration Office to complete any forms.
